If your next BOTOX® Cosmetic (botulinum toxin type A) treatment is between 3 and 6 months from the last treatment when your BOTOX® Cosmetic Benefits Card was swiped at the physician’s office, your card can be used toward payment of that treatment. A reward is only valid between 3 and 6 months from the last swipe at the same physician location that provided the card. Your card must be activated to be valid. Present your card at the time of treatment, and your $25 reward will be applied toward your treatment cost.
Each time your card is swiped at your physician’s office, your card is reauthorized with $25 toward your next treatment. You can receive those rewards once every 3 to 6 months until the card expires.
If it has been longer than 6 months since the last swipe, the card may still be valid for future rewards, although the $25 reward has expired. Bring the card to your next BOTOX® Cosmetic appointment and ask your healthcare provider to swipe the card to add a new $25 reward to be used toward your next treatment.
The BOTOX® Cosmetic Benefits Card is exclusive for the use of the recipient and the person who activated it. You must cancel the card immediately if it is lost or stolen. Instructions can be found at the Lost/Stolen link on this site.
This program expires on the last day of the month embossed on the card. The BOTOX® Cosmetic Benefits Card program may be changed, revoked, or discontinued at any time. Notice of any change and program expiration date will be posted in the My Account section on this site. The card is issued by Key Bank. No cash access. Misuse of debit cards is a civil offense. The issuers shall not be liable to the user for losses, expenses, claims, or other damages which the user may incur as a result of the card. Terms and conditions of use are subject to MasterCard® and Key Bank guidelines, which can be found at www.accountholder.net/TMG. Not valid for patients reimbursed by federal healthcare programs including Medicare, Medicaid, CHAMPUS, the Department of Veterans Affairs, state maternal and child health block grant programs under 42 U.S.C. section 701 et. seq., state social services block grant programs under 42 U.S.C. section 1397 et seq., or any other similar federal or state healthcare program; or for patients in MA and MI except for cash paying patients. Void where prohibited by law, taxed, or restricted. The BOTOX® Cosmetic Benefits Card is issued by Key Bank, N.A., pursuant to license by MasterCard International.
